The goal of the Assessment Evaluation and Review Services (AERS) Program is to help county residents to remain safe at home by connecting them with services and supports that can encourage their independence and personal well-being. This often means that individuals can live at home longer without having to consider moving to a nursing home or other facility placement.
How it works
A Registered Nurse from the AERS staff will complete an on-site visit (home, hospital, nursing home, etc.) and an assessment of the overall situation. Staff may ask questions about health history and diagnoses, current medical concerns, behavioral or mental health issues, social or family supports, environmental concerns and general financial or legal issues. All of these factors are considered to develop a plan of care with recommendations to connect the individual with resources and supports. Each person chooses whether to follow up on recommendations made by staff. There is no charge for an assessment.
